Lost csr card

Hey could anyone on here maybe help me with something?I have misplaced my csr health and safety card and not for the love of me can i remember where i put it or where it might be,and i have been searching for numbers all morning online to try and ring and get a replacement card,i thought it would have been straight forward enough,has anyone ever had any experience of this before?

Have you tried praying to St Anthony? I'm not religious but I prayed to him once when I lost my shoes. I didn't find them but I found loads of other stuff that didn't belong to me.
