Anyone else have to fill this out and is anyone else having  problems with it?
I have to fill it out every Saturday so I can bring the youngfella up to training. Can't get anything to load on the site and unable to submit the form.

Just checked there and it's just loading up a blank screen. Must be something wrong with the system.

U6 academy. Not really training as such.

Anyway turned up this morning and loads of parents also had problems signing up so they made us sign a form to say all was OK with the kid we had there.

We just finished up 3 weeks ago. Was too much hassle
