Just watched it.

That programme was interesting, particularly the Inmarsat stuff. The security vulnerability of the E&E hatch outside the cockpit door is also something I never knew about. That is embarrassing and I am not surprised that Boeing & Malaysian Airlines wouldn't comment.  That leaves a lot of possibilities wide open. I was thinking it definitely had to be the Captain until I saw that.

Regarding solutions, it always comes down to money. For example it seems incredible that the USA doesn't have radar covering all of mainland US airspace. That is simply down to cost. Flying could be a lot safer, but someone has to pay for it.

Heh?  When was the last time a plane crashed due to lack of radar coverage in the US?

Lack of coverage doesn't cause crashes. But coverage can help to prevent them.
