Recent business writing over here will tell you there is to be nothing after the name. Also it is no longer acceptable to write th, rd or st after a date, the date should be in full I.e 14 August 2013.

I was reading that all right Muzz. No requirement for a comma at all. Seems strange to me, and I wonder if it's a result of people getting lazy and all the shortened words and acronyms coming in.
