Question about extra time

Do teams have to line up with the conventional 6 backs, 2 midfielders and 6 forwards? Teams always do even when they've a double sweeper strategy. Just find it odd that these teams are giving the opposition potentially two attacks against a weakened defence (as in weakened compared to how their defensive structure is meant to be).

No reason why you couldn't have 8 lads in the backs though before the ball is thrown in.

Yes there is. You have to be in your position as per a standard team line out.

There was an incident recently in a club championship game in Mayo where one manager asked the ref if all his 15 players could start the 2nd half in their own half - the ref said no, that they have to start in their positions.

I'm not sure of any changes since but I remember a Mayo county final.... I think it was Shrule v Crossmolina and Crossmolina and they picked up two red cards in normal time and Trevor Mortimer picked up  a yellow himself and when it went to ET Crossmolina got the two players back but the yellow carried through and he got a second yellow in extra time... turned from a 15 v 13 to a 14 v 15.

Crossmolina won that match
You can forget about Sean Cavanagh as far as he's a man!

 The match I'm referring to is the 2005 county final, but, come to think of it I think that happened in a different game

Crossmolina won that match

In extra time you can go back to the full compliment, but it has to be two different players, not the lads sent off and IIRC that would include black cards and two yellows.
