Blomquist and Lunjberg.

I still say Manniger and Kuszcak.

From wikipedia:

QuoteAlthough Kuszczak didn't play enough Premiership games in the 2006-07 season, he received a winners medal by special application to the Premier League.

I just checked and as it turns out both Manniger and Kuszckak did recieve medals on special dispensation (I thought it was just the GAA that had loopholes).

Sorry about that lads.   :(

And yes Roy Carroll is the last answer on my list (Gillespie never won a Premiership)

Schmeichel
Diego Forlan
Essien
Park Ji-Sung
Dwight Yorke
Gudjohnson
Kanu
Luzhny
Carroll
Wreh
(Manniger)
(Kuszckak)
