Vicky Phelan (43), from Annacotty in Co Limerick, is suffering from terminal cancer after earlier receiving a negative smear test under the HSE's CervicalCheck programme.

On Monday, the HSE confirmed a total of 162 women were not informed of a delay in their cancer diagnosis.

A total of 17 of these women have died, but the cause of death is not yet known. The HSE is still trying to contact another 13 women.
She said the inquiry should examine why the HSE fought against her High Court case so hard, including asking for details of how sick she was and how soon she might die.

"It's upsetting to think they demanded proof that I was likely to die earlier than expected. I mean Jesus, how dare they?" Ms Phelan asked.

In January of this year, she was given between six and 12 months to live.

She is currently on a treatment drug which costs €8,500 per dosage every three weeks. She said she is currently paying for the drug herself.

It is all very Kafka esque.  She finds because while waiting for a consultation, she is given her file and takes a sneak peak.  Seeing some words she doesn't understand, she takes a photo with her phone so she can look it up later on.

I worked for the National Cancer Screening Service. Was also a direct report to Tony O'Brien, he was CEO at the time.

It didn't surprise me when this broke, consultants have been know to retrospectively change false negatives back to positives.

As a society we don't do accountability and always looking to pass the buck.

Tony O'Brien should resign immediately.

It was not at all surprising to hear that Mr Justice Cross, the judge who heard her case, described Vicky as the most impressive witness he had ever come across in his long career.

When the programme ended, Vicky, myself, my wonderful co-presenter David McCullagh and the rest of the Prime Time team went to our Green Room where we had a few drinks and chatted until late. It was a special time. I think everyone felt like me, that we were all in the presence of greatness.

In a note to the Minister for Health Simon Harris at the start of his report, the author, Dr Gabriel Scally, said there were many indications that the system in place "was doomed to fail at some point".
The report published on Wednesday, says there were serious gaps in the governance structures of the screening services.
"In the specific case of CervicalCheck, there was a demonstrable deficit of clear governance and reporting lines between it, the National Screening Service and the higher management structures of the HSE.
"This confusion complicated the reporting of issues and multiplied the risks. It is clear that there are also serious gaps in the range of expertise of professional and managerial staff directly engaged in the operation of CervicalCheck. There are, in addition, substantial weaknesses, indeed absences, of proper professional advisory structures. These deficiencies played no little role in the serious issues that concern this Scoping Inquiry."
The report said that "unlike many similar problems in healthcare delivery, and screening in particular, it was impossible to narrow the focus to a few or even a small number of areas".
"The problems uncovered are redolent of a whole-system failure", the report states.
The Scally report says the current policy and practice in relation to open disclosure in the health service was "deeply contradictory and unsatisfactory".

It said that in essence there was "no compelling requirement on clinicians to disclose" . It said it was left up to their personal and professional judgement.
There was, unfortunately, limited public health medicine input into CervicalCheck and I firmly believe that was to its detriment. The time has surely come where public health physicians are accorded the same recognition as clinical colleagues and their skills deployed at the core of all public health programmes. I hope that movement on this matter can take place in the near future."

Has the HSE ever been fit for purpose?

I worked for the NCSS for two years, some fantastic people on the ground but having to work with consultants was an eye opener.

We developed a GUI to work in-conjunction with digital mammograms, it was to replace the paper based traditional approach. A consultant would fill out the digital form instead of a paper form. They refused, citing change in work practice and wanted more money, they were already on €250K a year excluding what they earned in their private practice. The off-shot was they still filled out the paper and a nurse to had upload the info into the digital form.

Arrogant wankers, the only difference between a medical consultant and God is that God doesn't think he's a consultant.

My favourite quote was when asked if there was a cover up by the hse he said No, as the hse would not be competent enough to organise a cover up. I'm sure zero heads will roll as usual.

The head of the HSE resigned over this back in May.
